Insurance coverage is determined by your individual plan. Copayments, deductibles, coinsurance, noncovered services, and denied claims remain the patient’s responsibility.

- Outside laboratory, imaging, pharmacy, specialist, and hospital charges are billed separately by those organizations.
- Vaccines and immunizations are not included unless specifically stated.
- Addressing new medical concerns during a preventive examination may result in an additional office-visit charge.
- Uninsured and self-pay patients may request a Good Faith Estimate of expected charges before receiving scheduled services.
